Because the academy is intentionally small and active, we cannot promise that loaner clubs will be available for every participant.

The $300 fee covers all four scheduled academy sessions. A missed class does not create an individual private makeup lesson; Woods to Wedges plans to coordinate one group makeup opportunity for registered juniors who miss a session.

I am the parent/legal guardian of the junior golfer named on this form, or I am otherwise legally authorized to register the participant. I voluntarily authorize the junior's participation in the Woods to Wedges Fall Junior Golf Academy. I understand that golf instruction and practice involve inherent and ordinary risks, including but not limited to being struck by golf balls, golf clubs or equipment; slips, trips or falls; physical exertion; and contact with facility surfaces, training aids or other participants. I agree that the junior will follow instructor directions and facility safety rules. To the fullest extent permitted by applicable law, I knowingly assume the ordinary risks of participation and agree to release and hold harmless Woods to Wedges, its owners, employees, instructors, agents and the owners/operators of the host facility from claims arising from ordinary risks inherent in participation, except to the extent a claim cannot legally be waived or results from conduct for which liability cannot lawfully be released.

In the event of an emergency, Woods to Wedges will first attempt to contact the parent/guardian and emergency contact listed above. If neither can be reached promptly, I authorize staff to contact emergency services and obtain reasonable emergency assistance for the participant. I understand that Woods to Wedges does not provide medical care.

The academy is limited to a maximum of 8 junior golfers. Registration closes October 14th, 2026, or earlier if all spaces are filled. There are no additional academy fees.
